Self-loathing is beating ourselves up, a form of self abuse that there is a logical, reasoned, treatment for. First, it is my first time through life, so I can expect to make mistakes. Second, it is not the event, but my thinking about the event, that causes me these concerns. I can change my thinking.
I have power over only my will. I may have some influence over my thinking and actions, but not control or power. All else is beyond my power and control; it is none of my concern. What you do, think, do not do or say is none my concern, none of my business. That are your issues. Is what I am self-loathing over in my power or influence or none of my concern?
The mind is composed of a number of parts that communicate with each-other. The memory or knowledge; ego, a construction of mind, and the reasoning part and consciousness are separate parts. We may be doing what we think is our best action, and someone takes offence. That is their problem. We need to get along to survive, be part of a community, but also have a bit of space. We have too dense of population here, that is all there is to it. This planet cannot support the beautiful too many. Nature will decide who dies off and who remains. Oh, well, shit happens.

cephalic hyperinsulinemia
Is there such a thing as cephalic hyperinsulinemia?
Hyperinsulinemia is too much insulin, usually related to insulin resistance, but is there any need to associate it with insulin resistance or is just a diet high in carbohydrates enough?
Insulin is released in three phases, cephalic, postprandial, and basial. So what would be the symptoms of cephalic hyperinsulinema? Hunger at the site of food... the physiological part of tempation? Quick to hunger, always ready to eat, overweight likely. Sounds like me.
When I asked my GP, he said, perhaps, but we have no treatment, therefore it is not a recognised condition. WHF. I have not been back, but then I am now low carb.

Everywhere I look, I find Nuts! The internet is full of them, for the lack of a better word. I am one some days. It is time to do a inventory of self, to see what is there, and to provide a foundation for a change in direction.
At this point in my life I am still a bit heavy, but I am down 56 kg from my top weight. I know how damaging sugar, wheat, grains in general, omega 6 oils, exorphins, endocannabinoids, and hormones are. I know how to eat clean of these. I also know that 30 grams of carbohydrate leave me craving, but 55 works well. I can avoid hunger and cravings and maintain my weight.
I know that this is not the complete story of obesity, there is a mental portion... be it philosophy, psychological, social, or what ever handle you want to attach. Some of what I think is wrong, some has been corrected to better concepts, but is still producing cognitive discord. So what should I do? That is the question. Change of philosophy
Stoicism upheld the pantheistic view that the pure being of God is present in all material things. -or- However, because God is pure reason, everything happens rationally, according to a universal order.
If god is reason, then it is conceptual, and cannot be present anywhere... so god a concept of the sum of all concepts that describe all not yet understood phenomenal. Logos, the laws of nature, are everywhere, even in ELMM, which, to be biologically correct must be ELCMM. Eat less carbs, move more. Once we get the carbs down, and fat up with reasonable protein [(height(m)) squared x 20 = grams of protein], now our weight becomes dependent on the amount of fat we eat.
Ah well, life is just short and then we die. It does not mater anyway, who will remember anyway.

Likely Model
All this science and the model is still not right.
It is opinion only. My opinion is as good as there's so it should go something like this:
Excess Carbohydrates > Liver insulin resistance > muscle cell insulin resistance > hyperinsulinema > weight gain > cravings > low FFA, ketones > uncontrolled hunger, eating > obesity > beta cell death > type 2 DM
It does not matter, the only solution to all this is a low carb diet, to get the insulin down, and free the fat for utilization.

More Fat Chance

On page 9, Robert lists a bunch of factors in obesity. His list is conspicuous missing personal philosophy as a major factor. Once we decided that some items are not food, the problem becomes much smaller.
It is not the situation that causes the problem, but our judgement about the problem, that is the issue. Our personal philosophy can change, and we can obtain some relief from problem. If we eat to sate emotions, understanding that we a control only our will and all else is someone else's problem will provide relief. Once again... personal philosophy... that have changed so much since I was young. We were grateful for a warm bed some nights, and now, to be satisfied, what do the young want? Desires, greed, is the issue. We need to let go of excess and move back to minimalism in some decant form.
Satisfaction is success divided by expectations. If we philosophy lower our desires and expectations we will experience satisfactions and contentment. Contentment is a virtue. Thank you.

I think this should start high carbohydrate (not high fat), gets insulin flowing, and it is then about right. According to Lustig, 40% have sugar issue, 20% acellular carbohydrate processed food and the remainder are often hyperinsulinemic.
Also insulin resistance does not happen at once, but rather liver first, and glucose overflows to the body, and then muscle insulin resistance occurs. When fat cell insulin resistance occurs you have got a problem.
